White Arrow LLC - Accessorial Table and Policies
The CUSTOMER/CARRIER will be responsible for the following accessorial charges to the extent incurred:

| Diversion/Reconsignment | TL or IMDL $200.00 | $200.00 flat charge per occurrence. A diversion or re-consignment is any change of consignee, consignor, route, or destination made while the shipment is en route to its original consignee and destination, or any other change that requires a change in billing or additional movement of the transportation equipment. Charges will apply to the rate. Applicable from the origin to the original destination, applied to all actual miles, plus a re-consignment/diversion fee of $200.00. |

Charges apply if the Carrier arrives on time and after the free period, at a rate of $75.00 per hour, with billing in 15-minute increments of $18.75. |

| Redelivery | -//- | If upon delivery to the consignee, a shipment is unable to be received until a later time/date, Carrier will review the situation and take the appropriate measures (redelivery, return, etc). Hourly detention charges will apply if redelivery is to happen on the same day. Redelivery charges will apply if redelivery occurs the next day. The Carrier will provide the redelivery rate and authorization is required prior to rescheduling the redelivery. |

IMPORTANT! In addition to the applicable transportation rates, transportation services are subject to the following charges on all additional services, CARRIER will endeavor to provide when accessorial charges are incurred, but unless otherwise agreed to by the CARRIER in a written agreement signed by an authorized representative; CARRIER will not be required to provide notice of any accessorial charges. CARRIER may provide notice of these charges by fax, email, phone or by website posting. CARRIER's failure to notify does not release the obligation to pay. White Arrow may revise accessorial charges detailed within this Appendix upon providing a minimum of thirty (30) days advanced notice.

Authorization to Run Credit: By signing below, I/We authorize White Arrow, LLC and its affiliates to obtain and verify credit information from any source, including credit references and credit reporting agencies. White Arrow, LLC, and its affiliates are also authorized to respond to inquiries regarding our credit history with them.
Credit Terms: In consideration of credit extended by White Arrow, LLC and its affiliates, I/We agree to White Arrow's Rules and Regulations, available at whitearrow.com/rules-and-regulations, and all shipments are subject to them. Invoices will reflect payment terms. Failure to pay within the agreed terms may result in the removal of any discounts and the entire balance becoming immediately due and payable. If any balance is referred to an attorney or collection agency, I/We agree to pay reasonable attorney fees, court costs, and expenses associated with collection efforts. I/We agree to pay late charges at the maximum rate allowed by law, calculated from thirty days after the invoice date on any unpaid balance. I/We agree that this agreement is governed by the laws of the State of California and submit to its jurisdiction for all matters related to the collection of our account. White Arrow, LLC reserves the right to refuse or withdraw credit privileges at any time without prior notice, except as required by law.
